Note: When you click on the black “Submit” button above, your completed application should show up on your screen. If you are prompted to do so, provide missing information and click on “Submit” again. If you want to change an entry, click on the blue-green type that says “go back”, then start over. If your application is complete and correct, move on to Step 3.
Pay with your credit card or PayPal account by clicking on the payment logo below.
A PayPal account is NOT required.
Note: Only click the payment logo once or you will be charged for more than one membership! Once directed to the payment site, make sure the “Quantity” box for your request is set to 1. To pay with your credit card, click on the gray “Check Out” button. To pay with PayPal, click on the yellow “PayPal Check Out” button.
